The contractor will abstract case information, conduct informant interviews, present cases to the NYS Maternal Mortality Review Board, and participate in regular meetings. • One year experience conducting informant interviews on sensitive topics
• Three years experience conducting medical record reviews
• One year experience requesting information from facilities and providers
• One year experience in abstraction and data entry
• Cannot be a NYS health care facility or affiliate

• Obtain medical records from hospitals and other facilities
• Abstract case information into comprehensive data systems
• Conduct informant interviews with family members
• Present cases to the Maternal Mortality Review Board
• Review potential maternal deaths and select maternal morbidity cases
• Complete case summary forms
• Participate in Board meetings and required conferences
• Submit weekly status reports on case abstraction
• Securely manage sensitive patient information
• Perform OB/MFM physician reviews of potential cases
