- Design survey methodology for representative sampling of county residents
- Ensure adequate representation of rural populations in the county
- Conduct survey with at least 1,200 completed responses
- Develop Population Health Data Tool with local-level statistics
- Gather community input through qualitative research methods
- Analyze survey data and weight for representativeness
- Provide data in Excel format with data dictionary/codebook
- Generate crosstabs by geographic zone, gender, race, and other demographics
- Create comprehensive CHNA report with executive summary
- Identify community health priorities based on data analysis
- Provide regular progress updates to SCCHD staff
- Present data visually through narrative, tables, and graphs
- Ensure sample represents three defined county regions
- Include core BRFSS questions plus 15 optional module questions
- Analyze both quantitative and qualitative data sources
- Integrate local hospital and community data
- Provide all raw data and final reports
- Develop qualitative data codebook for community input
- Consider infographic display for key information
- Utilize existing health assessment data sources
