Buyer seeks to establish a contract for providing a comprehensive Weight Management Program for eligible employees, retirees, and their dependents. The program must include clinical care management, access to weight loss medications, and supportive services to help participants achieve and maintain healthy weight. Buyer requires the program to be scientifically-based, include coaching support, and offer both clinical oversight and practical tools for sustainable weight management.

• Have minimum five years experience performing similar weight management services
• Operate full-time, permanent business address with email and telephone availability
• Provide scientifically-based weight management practices with documented approach

• Provide comprehensive weight management program with care management services
• Facilitate access to weight loss medications with appropriate clinical oversight
• Coordinate care with WCPS health clinic and existing medical plan
• Deliver virtual and/or in-person provider access for program participants
• Provide coaches to support members in their weight loss journey
• Implement meal planning and nutrition education resources
• Offer online/mobile-based weight management tools like calorie trackers
• Create community connection opportunities for program participants
• Provide quarterly and annual utilization reporting and program analysis
• Conduct marketing and education campaign for employee enrollment
