Buyer is seeking a qualified medical care practice to provide an Advanced Practice Provider for the Community Outreach and Support Team (COAST) Program. The provider will enhance community-based healthcare services by improving access to primary care, reducing unnecessary emergency department visits, and offering specialized medical interventions. Services include primary care, wound care, substance use disorder management, and on-scene responses with EMS teams. The program aims to support individuals experiencing opioid withdrawal by offering buprenorphine and ensuring seamless transitions to treatment programs.

• Two years' experience as a medical care practice
• Advanced Practice Providers must have minimum two years' experience
• Mechanism for recruiting, hiring, credentialing staff
• Mechanism to ensure providers attain CME credits
• Electronic Medical Record system for documentation
• Ability to generate HIPAA-compliant reports
• Ability to participate in CRISP health information exchange
• Infrastructure to submit insurance claims

• Provide interim primary care for individuals referred by EMS
• Prescribe buprenorphine and coordinate care for opioid use disorders
• Offer evaluation, treatment, and follow-up for acute/chronic wounds
• Accompany EMS teams to low-acuity 9-1-1 calls
• Provide on-call services for MOUD consultations
• Complete patient assessments and medical screenings
• Participate in street outreach and home visits
• Document patient encounters in EMR system
• Generate monthly patient encounter reports
• Submit claims to insurance providers
• Participate in quality improvement initiatives
• Provide health education to program participants
• Complete minimum patient encounters (20 in year one)
• Collaborate with EMS clinicians and stakeholders
• Initiate referrals to medical and addiction treatment programs
