Buyer will fund up to six organizations to hire and train community health workers who will provide culturally competent services to residents in 15 specific zip codes. • Collect data on health outcomes and participant satisfaction
• Develop program integrity policies to prevent fraud and abuse
• Establish partnerships with local service providers and organizations
