• Assist injured fire department individuals in navigating healthcare post-injury
• Schedule medical appointments with primary care, specialists, and therapists
• Reduce time to definitive diagnosis and specialist appointments
• Provide a single point of contact for each injured individual
• Collaborate with City's Workers' Compensation process
• Work with injured individuals' private health insurance providers
• Eliminate unnecessary or duplicate medical appointments
• Keep individuals informed at every stage of care
• Assist with scheduling diagnostic testing and imaging
• Coordinate related follow-up services
