Buyer seeks proposals for the 2026 Current Unmet Needs Program to support longer-term, ongoing programs that address unmet community needs where limited funding is available. Programs should demonstrate community partnerships, non-duplication of existing services, and clear outcomes. Buyer will fund services for the fiscal year October 1, 2025, to September 30, 2026. Successful applicants must document program sustainability and provide in-person presentations to the DHHS Board.

• Provide solutions for unmet community needs with limited funding alternatives
• Deliver ongoing, longer-term support programs addressing community gaps
• Detail program outcomes for requested funding and past service outcomes
• Partner with community organizations to avoid service duplication
• Document how the specific community need was identified
• Serve defined client populations with appropriate eligibility requirements
• Conduct outreach, intake, and assessment processes for eligible participants
• Implement program evaluation activities to determine effectiveness
• Pursue additional funding sources to support program sustainability
• Present detailed program outcomes to DHHS Board in person
