Older Adult Falls Prevention Innovations

Buyer seeks to build upon its current falls prevention infrastructure by exploring new strategies to help older adults avoid falling and sustaining fall-related injuries. Buyer will fund one innovative project that builds on research, best practices, and evidence-based falls prevention programming. The project must address specific challenges in falls prevention, include a detailed implementation plan, and include evaluation components. The target population is adults age 60 and older, with a focus on decreasing fall risk factors or incidence.

- Collect and evaluate program outcome data
- Collaborate with community partners and stakeholders
- Provide culturally sensitive, person-centered services
- Present progress at Falls Free CT coalition meetings
- Develop interventions for high-risk older adults
- Customize programs for underrepresented older adult groups
- Identify barriers to falls prevention adoption
