Buyer seeks qualified consultants to provide senior services and/or senior community navigator services. The project includes two categories: Category A for senior services (in-home health, social engagement, caregiver support, etc.) and Category B for community navigators providing personalized case management. Buyer aims to support Minneapolis older adults so they can remain in their homes and communities. Services must be culturally appropriate and provided in the language of preference.
