Buyer is soliciting proposals from suppliers interested in providing Program of All-Inclusive Care for the Elderly (PACE) organization services. PACE provides comprehensive medical and social services to eligible individuals who live in the community. The buyer intends to establish PACE programs in multiple service regions across Georgia. Successful applicants will be granted permission to begin the PACE application process with CMS that will result in a three-way agreement.

- Financial viability and resources to operate PACE program
- Historical commitment to serving elderly population
- Ability to complete CMS application within 19-24 months
- Capability to establish service centers in awarded regions
- Establish PACE centers in awarded service regions
- Complete CMS application for PACE approval
- Provide comprehensive medical and social services to eligible participants
- Establish an interdisciplinary team for participant care
- Secure property/facilities in awarded service regions
- Provide all Medicare and Medicaid covered services
- Coordinate 24-hour care delivery
- Implement Electronic Medical Records system
- Create marketing and enrollment plans
- Achieve operational readiness within 19-24 months
