• Housing development must be fully occupied by September 30, 2025
• Cannot be scattered site development

• Co-locate healthcare services at housing developments
• Provide care coordination to improve health outcomes for residents
• Develop partnerships between housing providers and healthcare entities
• Implement healthcare programs targeting low-income households
• Track and report on health and housing outcomes over 3-year grant period
