Buyer seeks proposals from individuals and organizations to assist with implementing the Self-Determination Program for persons with developmental disabilities. The program provides participants with an individual budget to purchase services and support needed to achieve person-centered plans. Buyer seeks support in areas including outreach, orientation, recruitment of facilitators, and coaching for transitions to SDP. Services must be provided in both English and Spanish across Kern, Inyo, and Mono Counties.

• Develop mass marketing campaign for Self-Determination Program
• Create accessible videos explaining SDP in plain language
• Provide support for participants transitioning to SDP
• Assist with individual budget and spending plan development
• Conduct joint training for participants, families and staff
• Organize resource fairs and community outreach events
• Recruit and train Independent Facilitators
• Provide orientation support for new SDP participants
• Develop materials in both English and Spanish
• Facilitate collaborative workshops for shared learning opportunities
• Mentor new Independent Facilitators
• Deliver training on person-centered planning approaches
• Support participants in selecting financial management services
• Assist with recruiting and hiring support staff
• Develop backup staffing plans for participants
• Organize Q&A sessions about Self-Determination Program
• Conduct vendor training sessions
• Provide outreach to underrepresented communities
• Collect and analyze outcome data
• Support clients in achieving person-centered plan goals
