Buyer seeks to establish a Multiple Award Contract with at least two providers per region for case management services. Buyer administers the Ohio Home Care Waiver program for individuals ages 0-59 with nursing facility level of care and the Specialized Recovery Services program for adults with severe mental illness or chronic conditions. Selected providers will conduct assessments, develop person-centered service plans, coordinate care, and monitor health and safety. Providers must collaborate with HOME Choice, Ohio Benefits Long-Term Services, and MyCare Ohio programs.

• Thorough understanding of all program glossary terms and requirements
• Ohio certified Minority Business Enterprise subcontractor required (15% minimum)
• Clinical Director must be RN or LSW/LISW with 5 years management experience
• Contract Manager must have bachelor's degree and 8 years management experience
• Two years operations experience with federally or state-funded programs

• Provide case management services for Ohio Home Care Waiver program
• Provide recovery management for Specialized Recovery Services program
• Determine level of care via ODM-approved assessment tools
• Develop and monitor person-centered service plans
• Coordinate multiple services across providers and payers
• Monitor health, welfare, and safety through regular contacts
• Assist with housing coordination and transportation needs
• Respond to individual crisis situations and reportable incidents
• Maintain acuity-based contact schedules for enrolled individuals
• Coordinate with Medicaid managed care organizations
