Buyer seeks proposals from nonprofit organizations to provide Long-Term Care Ombudsman Services. The program investigates and resolves complaints from residents in nursing homes and residential care facilities. The Ombudsman also provides consultation, education, and advocacy on resident rights and represents resident concerns on a systems level. Services will enhance quality of life and care for residents by addressing complaints, offering consultation, and advocating for their rights.

- 3 years experience providing LTC Ombudsman Services to target population
- Nonprofit organization with 501(c)(3) determination
- Willing to comply with City contracting requirements
- Ability to become certified vendor within 10 days
- Investigate and resolve complaints from long-term care facility residents
- Provide witnessing services for Advanced Health Care Directives
- Represent residents' interests before governmental agencies
- Maintain at least 20 certified LTC Ombudsman volunteers
- Monitor both nursing facilities and residential care facilities regularly
- Attend Resident and Family Council meetings
- Provide information and assistance to facilities staff
- Conduct community education sessions on LTC issues
- Document services in required state and local databases
- Advocate for systemic changes to protect residents' rights
