Statewide Medical Chart Audit of CCORP

The buyer seeks a contractor to perform a medical chart audit of the California Cardiovascular Outcomes Reporting Program (CCORP) Coronary Artery Bypass Graft (CABG) data. The contractor will conduct on-site or remote audits of medical charts at hospitals throughout California. The contract period is from June 1, 2025 to May 31, 2028. The contractor must have qualified staff including medical chart auditors, registered nurses, and at least one physician.

• 50% of re-abstracting staff must be registered nurses
• 50% of re-abstracting staff must have 5+ medical chart re-abstraction projects in last 10 years
• Staff member with 5+ medical chart re-abstraction tool development projects in last 10 years
• Database management staff with 5+ years experience
• At least one licensed cardiologist or cardiac surgeon on staff

• Conduct on-site or remote access audits of medical charts at CCORP hospitals
• Perform peer re-abstractions on 5% of charts
• Have physician review discrepancies in peer re-abstracted charts
• Complete audit of 1,250 total charts at 18 hospitals each year
• Maintain reliability of chart re-abstractions and overall quality control
