- Facilitate coaching sessions tailored to each practice's needs
- Develop timelines and estimate coaching hours required
- Create educational materials to help practices interpret milestones
- Analyze practice progress using assessment surveys
- Hold monthly meetings with community health centers
- Attend weekly meetings with CalOptima Health
- Stay current on EPT program requirements
