• Must be incorporated as a non-profit or other eligible entity type
• Demonstrate capacity for adequate administrative and accounting procedures
• Have a current Federal Tax ID number
• Ability to meet project expenses in advance of reimbursement
• Staff must include parent/family member of individual with developmental disability

• Provide family connection and support services
• Offer information, education, and referral assistance
• Promote self-advocacy and self-determination for individuals with developmental disabilities
• Facilitate system improvement activities
• Conduct outreach to diverse communities
• Organize social and networking opportunities for families
• Develop and distribute written information about local services and events
• Support inclusion of individuals with developmental disabilities in community activities
• Participate in community advisory and leadership groups
• Evaluate program services and priorities annually
