Buyer seeks to award contracts for Housing Financial Assistance for individuals and households who are homeless or at imminent risk of homelessness and participating in CARE Court services and/or Behavioral Health Bridge Housing. The program will provide one-time, short-term, or mid-term financial assistance to stabilize housing situations. Funding is available through the California Behavioral Health Bridge Housing Program. Buyer aims to assist people experiencing homelessness who have serious behavioral health conditions.

- Currently certified to use Alameda County's HMIS or have comparable database experience
- Meet funding requirements for BHBH Program elements
- Possess all necessary permits, licenses, and professional credentials
- Provide housing financial assistance to eligible individuals and households
- Conduct community outreach about the financial assistance program
- Create and implement an application process for financial assistance
- Develop housing stability plans for applicants
- Administer short-term and mid-term financial assistance
- Create service plans for participants addressing behavioral health and housing needs
- Provide referrals to supportive services
- Ensure access to medication assisted treatment and psychotropic medications
- Enter participant data into the Homeless Management Information System
- Submit quarterly narrative and data reports on program progress
