The buyer is seeking Audiologists to provide timely on-site audiological services for Preakness Healthcare Center. The services must comply with Federal and State Regulations for subacute and long-term care facilities. The buyer requires audiologists to perform assessments, hearing tests, and make recommendations for ENT evaluations. The contract period is from January 1, 2025 through December 31, 2025.

• Audiologist, Certification by the American Board of Audiologists, licensed in the State of New Jersey
• Minimum of five years' experience in treatment of subacute and long-term care residents
• Evidence of Professional Liability Insurance: $1M Occurrence /$3M Aggregate
• Vaccination as mandated by the NJ DOH, CMS or regulatory agencies

• Perform assessments and follow-up visits pursuant to physician order
• Arrange coverage during planned absence
• Perform hearing tests
• Make recommendations for ENT evaluation
• Make minor repairs to hearing aids
• Make molds for hearing aids; fit and dispense hearing aids
• Provide monthly on-site services on a pre-approved schedule
• Prepare, authenticate and date progress notes in resident's medical record
• Comply with all applicable Federal, State and local laws, rules and regulations
• Adhere to Facility Policies and Procedures including Infection Prevention and COVID-19 policies
