• Designated as Specialty Mental Health Care Management Agency in Broome County
• In good standing with Lead Health Home(s)
• Established working relationship with Broome County Adult Single Point of Access
• Utilizes Electronic Medical Health Record compliant with NYS guidelines
• Maintains adequate staffing capacity for care management and supervision

• Provide comprehensive care management services
• Assist individuals in achieving goals identified in person-centered Plan of Care
• Improve transitions between levels of care
• Provide linkages to appropriate services
• Increase communication among providers
• Reduce incidences of homelessness and justice-involved recidivism
• Deliver culturally competent, strength-based, trauma-informed services
• Assist individuals in obtaining Medicaid coverage
• Transition individuals to Medicaid-reimbursable Health Home Care Management services
• Participate in Adult Single Point of Access (A-SPOA) Committee meetings
