• At least two years of experience performing similar or relevant services
• Capacity to provide Project Services including Required Staffing

• Provide coordinated case management services to 35 individuals annually
• Conduct screening and assessment interviews using evidence-based risk and needs tool
• Develop treatment plans tailored to substance-affected parents and families
• Provide trauma-informed case management and ongoing case coordination
• Conduct urine and hair follicle testing and monitor alcohol monitoring devices
• Provide weekly supervised visitation for families referred to the FTC
• Collect and maintain data as required by SAMHSA
• Staff two full-time Case Managers and one part-time Court Appointed Special Advocate Coordinator
