Buyer seeks to provide in-home and community-based substance use disorder treatment services for Black/African American and Hispanic/Latinx populations in urban areas of Wisconsin. The program aims to reduce health disparities and improve behavioral health equity through culturally competent, evidence-based approaches. Services will include intervention, treatment, family support, and harm reduction strategies. Buyer intends to award grants to approximately two applicants to implement these services.

• Be a public agency or nonprofit organization
• Have certification under Wisconsin Administrative Code DHS 75
• Have capacity to provide at least one level of substance use disorder care
• Have experience providing services to target population

• Provide in-home and community-based substance use disorder treatment services
• Develop culturally competent strategies to engage Black and Hispanic individuals
• Address social determinants of health contributing to substance use disorders
• Implement harm reduction approaches to reduce overdose risk
• Provide comprehensive family support services
• Conduct outreach and education in Black and Hispanic communities
• Incorporate cultural healing practices into treatment services
• Coordinate care with other service providers
• Collect and report data on program outcomes
• Develop sustainability plan for services beyond grant period
