The program aims to reduce health disparities and improve behavioral health equity through culturally competent, evidence-based approaches. Services will include intervention, treatment, family support, and harm reduction strategies. Buyer intends to award grants to approximately two applicants to implement these services.

- Be a public agency or nonprofit organization
- Have certification under Wisconsin Administrative Code DHS 75
- Have capacity to provide at least one level of substance use disorder care
- Have experience providing services to target population
- Provide in-home and community-based substance use disorder treatment services
- Develop culturally competent strategies to engage Black and Hispanic individuals
- Address social determinants of health contributing to substance use disorders
- Implement harm reduction approaches to reduce overdose risk
- Provide comprehensive family support services
- Conduct outreach and education in Black and Hispanic communities
- Incorporate cultural healing practices into treatment services
- Coordinate care with other service providers
- Collect and report data on program outcomes
- Develop sustainability plan for services beyond grant period
