Buyer seeks a qualified service provider to offer street outreach, case management, and rapid rehousing services for persons experiencing homelessness. The provider will use trauma-informed and person-centered approaches to connect individuals with resources. Services include street-based case management, housing navigation, and housing stability support. The contract is contingent on the buyer receiving Encampment Resolution Funding Program, Round 3 (ERF3) award.

• 9/2/2024 - Contractor Selection Date

• Experience working with persons experiencing homelessness using trauma-informed approaches
• Knowledge of housing programs and local homeless service programs
• Nonprofit organizations must be incorporated in California
• Ability to sign City's standard contract for services

• Provide street-based case management services within the encampment area
• Complete comprehensive Biopsychosocial assessments for clients
• Link clients to housing, medical, mental health, and other services
• Assist clients in obtaining identification and enrolling in benefits
• Provide ongoing case management and housing stability services
• Coordinate with City staff to identify appropriate housing options
• Assist minimum 125 households with rapid rehousing and housing stabilization
• Maintain communication with Vista COPPS unit and attend regular meetings
• Enter client data into Homeless Management Information System (HMIS)
• Develop recommendations for community and program improvements
