Behavioral Health Services in Primary Care for Older Adults

Buyer seeks a vendor to provide behavioral health services in primary care for older adults ages 55 and older. Services include case management, clinical assessment, and outreach in the Tenderloin and South of Market neighborhoods. The program aims to serve San Francisco residents with behavioral health and/or substance use disorders, including those experiencing chronic homelessness or at risk of homelessness. The vendor will develop and implement care plans addressing behavioral health, housing, medical care, and social services needs.
