Buyer seeks providers to deliver home care services to Medicaid-eligible individuals in New York City. Services include housekeeping, home attendant care, emergency services, and consumer-directed assistance. Providers must be licensed, have significant experience, and meet financial requirements. The contract term is 3 years with options to renew.

• 6/30/2024 - Proposal Due Date
• 7/1/2025 - Contract Start Date

• 10 years experience as direct provider of home care services
• NYS licensed home care services agency in accordance with 18 NYCRR 505.14
• Licensed in at least 3 NYC boroughs
• Serve an average of at least 100 clients annually
• Have $500,000 in working capital to support 100 cases without reimbursement

• Provide housekeeping services to Medicaid-eligible individuals
• Provide home attendant services to Medicaid-eligible individuals
• Provide emergency and exceptional home attendant services
• Provide Consumer Directed Personal Assistance Program services
• Conduct supervisory nursing visits every 90 days
• Conduct nursing assessment visits every 180 days
• Implement mechanisms to measure consumer satisfaction
• Establish a Quality Assurance and Improvement Committee
• Maintain accurate client and worker personnel records
• Bill clients for surplus income amounts and maintain accounts
